The Freeman Regional Health Services Foundation will host our 10th Annual Golf Tournament on June 14, 2019 at Valley View Golf Course in Freeman, SD. 

• Sign up now for this 4-person, 18-hole golf scramble.

• The entry fee is $75/golfer or $300/team and will be limited to the first 18 teams to register. 

• Individual golfers will be registered and placed onto teams. 

• Registration includes 18 holes of golf, shared cart, swag bag, lunch, and door prizes.

• Register by the June 5, 2019 deadline.
